How to set up a new affiliate profile

1) Go to /.htaccess line 6 and add the custom URL and sup_id into a new line. They should be the same, i.e. http://www.ellicksonusa.com/bobsdistributors should forward to http://www.ellicksonusa.com/draft-table/aff.html?sup_id=bobsdistributors

2) Go to /draft-table/aff.html line 334 and add the new sup_id and the affiliate's phone number into a new definition

3) Go to /draft-table/lib/send-contact-aff.php line 21 and add the affiliates's sup_id and email address as a new definition

4) TEST
